NHTSA Campaign: 19V741000

Component: AIR BAGS:FRONTAL:PASSENGER SIDE:INFLATOR MODULE

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2010-2016 4Runner, 2003-2006 Tundra, 2003-2013 Corolla, 2009-2010 Corolla Matrix, 2004-2005 RAV4, 2002-2007 Sequoia, 2011-2013 Sienna, 2008-2012 Scion xB, 2008-2009 Lexus IS-F, 2007-2012 Yaris and Lexus ES350, 2010-2017 Lexus GX460, 2002-2010 Lexus SC430, 2006-2012 Lexus IS250 and IS350 and 2010-2015 Lexus IS250C and IS350C vehicles. These vehicles had their driver or passenger frontal air bag inflators previously replaced under a prior recall using inflators of the same design. The inflators may explode due to propellant degradation occurring after long-term exposure to high absolute humidity, temperature and temperature cycling.

Risk: An inflator explosion may result in sharp metal fragments striking the driver or other occupants resulting in serious injury or death.

Fix: Toyota will notify owners. Depending on the vehicle model, dealers will replace the front driver inflator, front passenger airbag inflator, the airbag assembly, or the front passenger airbag sub-assembly and inflator. The recall began November 20, 2019. Owners may contact Toyota customer service at 1-888-270-9371 or Lexus customer service at 1-800-255-3987. Toyota's numbers for this recall are J0A, J0B, and J0C. Lexus' numbers for this recall are JLI, JLJ, and JLK.

NHTSA Campaign: 15V284000

Component: AIR BAGS:FRONTAL:DRIVER SIDE:INFLATOR MODULE

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2004-2005 Toyota RAV4 vehicles manufactured May 13, 2003, to October 25, 2005. The affected vehicles are equipped with a dual-stage driver frontal air bag that may be susceptible to moisture intrusion which, over time, could cause the inflator to rupture.

Risk: In the event of a crash necessitating deployment of the driver's frontal air bag, the inflator could rupture with metal fragments striking the driver or other occupants resulting in serious injury or death.

Fix: Owners of these vehicles are advised not to drive their vehicles until the recall remedy has been completed. Toyota will notify owners, and dealers will replace the driver's frontal air bag, free of charge. Owners were mailed an interim notice June 17, 2015. The recall began on August 21, 2015. Owners may contact Toyota customer service at 1-800-331-4331.

Your 2004 RAV4 is subject to a Takata airbag recall, and you've received multiple safety recall notices from Toyota since purchasing the car in 2019. You've called Toyota at least 8 times and been promised contact for airbag replacement, but Toyota has not followed up. Because you live on a remote Alaskan island without a local Toyota dealer, getting your vehicle to the nearest service location in Anchorage requires a 12-24 hour ferry ride plus a 2-8 hour drive. The fix would involve Toyota contacting you to schedule an appointment and replacing the recalled airbag system, though logistics may require shipping the vehicle or traveling to Anchorage.

You scheduled multiple appointments to have a safety recall on your airbags completed, but faced repeated service delays and scheduling issues at the dealership. After initially being told to return in 2 hours, your car hadn't been worked on when you came back, and subsequent attempts to schedule the recall were hampered by the dealership claiming they didn't have parts in stock, then failing to contact you when parts allegedly arrived five months later. The recall repair will require the dealership to install replacement airbag components once parts are available and an appointment is successfully completed.

You've been receiving recall notices for Takata airbags on a 2004 RAV4 you haven't owned for over 4 years and traded in at a Toyota dealership in October 2013. Despite contacting Toyota and the NH DMV with your vehicle's VIN, the recall notices continue to arrive with your wife's name (who passed away in 2015), and you're unable to get the registration records corrected to reflect the actual current owner. You're seeking clarification on how to fix the vehicle registration records so the current owner receives the recall notice instead.

Filed: Jan 2023

Your transmission slips suddenly while driving, including on the highway, which could be dangerous. The electronic control module that manages your transmission may be defective and causing this issue. Fixing this will likely require diagnosing and possibly replacing the transmission control module.

When you shift your RAV4 into Drive, the vehicle hesitates and all dashboard warning lights turn on with a "service needed" message appearing. Your dealer diagnosed a failing transmission and replaced it, but the same problem happened again. They then found the computer was faulty, though the vehicle hasn't been fully repaired yet. Fixing this will likely require computer diagnostic work and possibly replacement of the transmission control module or engine control unit.

Filed: Oct 2019

Your driver's side sun visor keeps falling down and blocking your view of the road, and it won't stay in the upright position. Other owners of 2004 RAV4s and later model years have reported the same problem. The fix will likely involve replacing the visor clip or hinge mechanism that holds it in place.

Your sun visors keep falling down on their own. When you remove them to avoid the problem, you get too much sun glare and reduced visibility even with sunglasses. Leaving them fully down also restricts your view of the road. You need replacement sun visors that will stay in place properly.

Filed: Jul 2020

You've noticed that the rear suspension components on your 2004 RAV4 are showing significant rust. While rust on suspension parts can eventually affect their strength and performance, there's no active recall listed for this issue. A mechanic should inspect the rusted suspension components to determine if they need replacement or if they're still structurally safe.

Filed: May 2017

You received a recall notice for your 2004 Toyota RAV4's suspension system, but the dealership couldn't get the parts needed to complete the repair. The manufacturer took longer than a reasonable timeframe to make the repair parts available, leaving your vehicle unable to be fixed under the recall.

criticalPOWER TRAIN,UNKNOWN OR OTHER,SERVICE BRAKES
Filed: Oct 2024

While driving on the highway and stopping at a red light, your brakes became unresponsive and your car continued rolling forward despite pressing the brake pedal hard. The gas pedal also became unresponsive. You were only able to stop the car by turning off the ignition. After restarting, the brakes and gas worked normally again. This is the second time this has happened—the first incident occurred in reverse—and even after having new brake pads, rotors, engine, and transmission checked by a mechanic, the problem recurred a few months later. The repair will likely require a detailed diagnostic of the brake system, engine control systems, and transmission electronics to identify why brakes and throttle become unresponsive.

criticalPOWER TRAIN,ELECTRICAL SYSTEM,UNKNOWN OR OTHER
Filed: Aug 2024

Your transmission started shifting slowly and jerking in lower gears, and your check engine light came on. A transmission specialist said the vehicle wasn't safe to drive because it could stall without warning, requiring a new transmission. During replacement, the technician found that your electronic control module (ECM) also needed to be replaced, which he indicated is a common issue on 2003–2005 RAV4s. The repair will involve replacing both the transmission and the electronic control module that controls it.

criticalELECTRICAL SYSTEM,ENGINE
Filed: Aug 2018

Your transmission is shifting hard, slipping between gears, and triggering warning codes about transmission solenoids. A transmission specialist has determined the root cause is a faulty engine control module (ECM), which is a known issue that Toyota has declined to replace. This unpredictable transmission behavior creates a safety concern, especially with children in the vehicle. Fixing this issue will likely require replacing the ECM (engine control module) to restore proper transmission operation.

While reversing your 2004 RAV4, the accelerator pedal became stuck and wouldn't release. The brake pedal failed to stop the vehicle, and it accelerated to over 55 mph before crashing into another car. The dealer couldn't identify the cause of the unintended acceleration. Addressing this issue would likely involve inspecting the accelerator pedal assembly, throttle cable, and brake system components to prevent recurrence.

Seat Belts1 issue
criticalSEAT BELTS
CrashFiled: Jun 2017

Your car was rear-ended at about 30 mph while stopped in traffic. You and your son were both wearing seat belts, but the seat belts didn't lock or restrain you properly during the collision, resulting in serious injuries including concussion, whiplash, and neck/shoulder/back injuries that required physical therapy. The failure of the seat belts to function as designed left you and your son without proper protection despite being buckled in. A seat belt system inspection and potential replacement of the seat belt pretensioners or retractor mechanisms may be needed.

Filed: Oct 2022

Your car's TRAC system (which automatically applies the brakes when it detects wheel slippage) is activating unexpectedly on curves even when you've turned it off. On wet roads, this has caused your car to skid out of your lane. The TRAC warning light comes on and beeps when this happens, but the issue occurs randomly and hasn't been reproduced by a mechanic yet.

Engine1 issue
highENGINE
Filed: Dec 2017

While driving, your check engine and VSA warning lights came on together, and your temperature gauge quickly rose, showing the engine was overheating. A mechanic found that the cylinder head bolt threads stripped due to stress between the aluminum cylinder block and head, causing the engine to leak coolant. Your engine is now at risk of overheating. Fixing this will require replacing the engine.

NHTSA #: 11053548
Electronic Stability Control (esc)1 issue
highELECTRONIC STABILITY CONTROL (ESC)
Filed: Feb 2017

When driving uphill or turning, your ESC (Electronic Stability Control) light occasionally comes on with three beeps from the dashboard, causing your car to jerk and lose power briefly before returning to normal speed. This has happened 5 times in the past month and is getting worse each time, making you hesitant to drive the car. Fixing this will likely require a diagnostic scan at a Toyota dealer to identify whether the problem is with the ESC sensor, wheel speed sensor, or the ESC control module itself.

Power Train,vehicle Speed Control,traction Control System1 issue
highPOWER TRAIN,VEHICLE SPEED CONTROL,TRACTION CONTROL SYSTEM
Filed: Jan 2017

Your VSC/TRAC warning light came on while driving, and your transmission began hesitating when starting from a stop and jerking forward, plus RPMs fluctuating at constant speeds. A transmission shop replaced your transmission for over $4,600, but the same warning light and problems returned within 2 weeks; now the engine won't restart after a diagnostic attempt. The issue may involve the transmission solenoid, engine control module (ECM), or both, and could require diagnosis with advanced diagnostic equipment to identify the root cause.
