NHTSA Campaign: 16V198000

Component: SERVICE BRAKES, HYDRAULIC:ANTILOCK/TRACTION CONTROL/ELECTRONIC LIMITED SLIP

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain model year 2016 Toyota RAV4 vehicles manufactured October 30, 2015, to February 3, 2016, 2016 Lexus RX350 vehicles manufactured October 26, 2015, to February 8, 2016, and 2016 Lexus ES350 vehicles manufactured September 1, 2015, to February 29, 2016. These vehicles are equipped with an ABS actuator that may have a damaged O-ring which could result in improper brake fluid pressure control during ABS, Traction Control, or Stability Control activation. As such, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard (FMVSS) number 126, "Electronic Stability Control Systems."

Risk: Insufficient brake fluid pressure may cause a loss of vehicle control, increasing the risk of a crash.

Fix: Toyota will notify owners, and dealers will inspect the brake actuator, replacing it as necessary, free of charge. The recall began on May 4, 2016, 2016. Owners may contact Toyota customer service at 1-800-331-4331. Toyota's number for this recall is G0I, GLC.

As a result, these vehicles fail to comply with the requirements of Federal Motor Vehicle Safety Standard number 110, "Tire Selection and Rims for Passenger Cars."

Risk: An incorrect label may result in the vehicle being overloaded, increasing the risk of a crash.

Fix: SET will notify owners, and provide them a corrected label to be installed over the inaccurate label, free of charge. The recall began on June 3, 2016. Owners may contact SET customer service at 1-866-405-4226. SET's number for this recall is SET16A.

NHTSA Campaign: 23V734000

Component: ELECTRICAL SYSTEM:12V/24V/48V BATTERY

Toyota Motor Engineering & Manufacturing (Toyota) is recalling certain 2013-2018 RAV4 vehicles. Certain replacement 12-volt batteries may not fit properly in the battery tray, allowing the battery to move and contact the hold-down bracket, possibly causing a short circuit.

Risk: A battery short circuit increases the risk of a fire.

Fix: Dealers will replace the battery clamp sub-assembly, battery tray, and positive terminal cover, free of charge. Owner letters were mailed December 27, 2024, to 2013-2014 RAV4 owners. Additional owner notification letters will be mailed in phases, phase 2 will begin on May 14, 2025, and phase 3 in late July 2025. Owners may contact Toyota's customer service at 1-800-331-4331. Your 2016 RAV4 failed to restart after parking, and when you opened the hood, you saw smoke coming from a melted battery with fluid leaking from it. No warning lights appeared before this happened. Your dealer confirmed this is related to an ongoing recall (Campaign 23V734000), but the replacement parts weren't available at the time, and your vehicle couldn't be repaired. The repair will likely involve replacing the battery and inspecting the electrical system for damage caused by the overheating.

You purchased a 2016 Toyota RAV4 from a private seller with 98,000 miles showing on the odometer, but a Carfax report revealed the vehicle actually had 170,000 miles recorded in November 2025—indicating the odometer may have been tampered with or rolled back. This is odometer fraud, which affects the true value and wear history of your vehicle. You may want to contact your local law enforcement or consumer protection agency to report this fraudulent transaction.

Filed: Jan 2026

Your dashboard is displaying a warning message that says your air bags are malfunctioning. This is a safety concern that requires immediate attention from a Toyota dealer. The airbag system needs to be diagnosed and repaired to ensure your safety features work properly in a collision.

CrashFiled: Sep 2025

You were involved in a low-speed collision (30-40 MPH rear-end crash) where your 2016 Rav4 was deemed a total loss by insurance. Multiple airbags deployed during the impact — both front airbags and side curtain airbags — causing facial injuries including a bruised eyebrow, bloody nose, and shoulder-to-elbow bruising from the side airbag. Your hood also opened on impact and a coolant hose began leaking. The repair or replacement of the airbag system, related sensors, and coolant system components would likely be involved in addressing this incident.

Seat Belts1 issue
criticalSEAT BELTS
Filed: Aug 2025

Your rear passenger-side seat belt became stuck and wouldn't unlatch, trapping your passenger inside. It took two people to help free your passenger, who had to climb over the back seat to escape. In an emergency situation like a fire or accident, this could have been dangerous or life-threatening. The seat belt latch mechanism will likely need to be replaced or repaired by a Toyota dealership.

FireFiled: Apr 2025

Your car caught fire under the hood while driving, with smoke billowing out at a traffic light. Your husband used his shirt to help extinguish the flames, but the fire caused extensive damage including a hole burned through the hood, melted plastic around the battery area, and a damaged radiator hose. The structural damage to the hood has compromised your vehicle's crash safety rating, and the car is now unsafe to drive. The repair will require replacement of the hood, radiator hose, battery area components, and a thorough inspection of the electrical and engine systems to determine the fire's cause.

Filed: Jan 2026

Your truck has been shutting off unexpectedly while driving and experiencing recurring electrical issues. After a battery replacement, your headlights began failing one at a time and dimming, while the check engine light keeps coming back on and the all-wheel drive system warning appears. Even after dealership repairs, the problems return within weeks, and you've noticed the engine is consuming fuel much faster than normal. The dealership will likely need to diagnose the electrical system, check the battery connections and alternator output, and run a full engine diagnostic to find what's causing the repeated warning lights and fuel consumption issue.

Filed: May 2025

Your 2016 Toyota RAV4 has an open recall, but you're unable to get it repaired because the vehicle is currently stored in England and was not originally sold there. Toyota dealerships in England cannot perform the repair due to lack of OEM parts and compatibility issues with a US-market vehicle. The military shipment service (Mildenhall VPC) is refusing to transport your car back to the United States until the recall is addressed, leaving you unable to resolve the situation while the vehicle remains overseas. Resolving this will likely require either contacting Toyota corporate for a waiver, having the recall cleared from your VIN remotely, or arranging private international transport.

Your headlights and taillights are becoming cloudy and hazy inside, which is dimming the light output and making it harder for you to see the road and harder for other drivers to see you at night. Filed: Sep 2025

Your RAV4's liftgate won't close properly using the electric buttons on your key fob or the buttons inside and outside the vehicle—you have to push it shut manually, and even then it sometimes takes several attempts to latch securely. The dealer says the hinges and liftgate assembly need to be replaced to fix the problem. This repair will likely involve removing and replacing the liftgate hinges and the liftgate panel itself.

Filed: Mar 2025

Your car may experience a shuddering or vibrating feeling while driving at light throttle speeds between 25–50 mph, caused by an issue in the torque converter. This shudder is noticeable but doesn't affect safety or cause the vehicle to lose power. The fix will likely involve servicing or replacing the torque converter and may require a transmission fluid and filter change.
