NHTSA Campaign: 12V537000
Component: STEERING
Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 5, 2003, through March 30, 2009. The affected vehicles have a steering intermediate extension shaft assembly built with metal splines that complete the connection from the steering wheel to the steering gear. These splines may have been manufactured with an insufficient hardness.
Risk: Over time, the splines may wear and eventually fail resulting in a loss of steering ability, increasing the risk of a vehicle crash.
Fix: Toyota will notify owners, and dealers will inspect the extension shaft and replace it if it is one with insufficient hardness, free of charge. The recall began during December 2012. Owners may contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331.
NHTSA Campaign: 12V536000
Component: ENGINE AND ENGINE COOLING
Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 6, 2003, through March 30, 2009 and FCHV-adv vehicles manufactured December 12, 2008, through September 13, 2011. During manufacturing, a scratch may have occurred inside of the electrically driven water pump at the coil wire. The coil wire may corrode at the scratched portion.
Risk: The corroded coil wire may break and the water pump could stop. The corroded coil wire could cause a short circuit between coil wires and a possible open fuse, creating a stall-like condition of the hybrid system while the vehicle is being driven. This may increase the risk of a vehicle crash.
Fix: Toyota will notify its owners, and dealers will replace the electric water pump, free of charge. The recall began on January 17, 2013. Owners may contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331.
NHTSA Campaign: 09V388000
Component: VEHICLE SPEED CONTROL:ACCELERATOR PEDAL
TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.
Risk: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.
Fix: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.
NHTSA Campaign: 06V096000
Component: AIR BAGS
ON CERTAIN VEHICLES, DUE TO IMPROPER ASSEMBLY OF THE AIR BAG INFLATOR, WHICH IS USED IN THE SIDE AIR BAG, THE CURTAIN SHIELD AIR BAG, AND THE KNEE AIR BAG ASSEMBLY, SOME INFLATORS WERE PRODUCED WITH AN INSUFFICIENT AMOUNT OF THE HEATING AGENTS NECESSARY FOR PROPER AIR BAG DEPLOYMENT. IN THIS CONDITION, THE EXPANSION FORCE OF THE GAS MAY BE INSUFFICIENT TO PROPERLY INFLATE THE AIR BAG WHEN THE SRS SYSTEM IS ACTIVATED DURING A CRASH.
Risk: THIS MAY INCREASE THE RISK OF INJURY TO THE OCCUPANT IN THE INVOLVED SEATING POSITION IN THE EVENT OF A CRASH.
Fix: DEALERS WILL REPLACE THE SPECIFIC SRS AIR BAG. THE RECALL BEGAN ON APRIL 6, 2006. OWNERS MAY CONTACT TOYOTA AT 1-888-270-9371, SCION AT 1-866-548-1851, OR LEXUS AT 1-800-255-3987.
NHTSA Campaign: 10V036000
Component: EQUIPMENT:OTHER:LABELS
SOUTHEAST TOYOTA IS RECALLING CERTAIN MODEL YEAR 2005-2011 PASSENGER VEHICLES FOR FAILING TO COMPLY WITH THE REQUIREMENTS OF FEDERAL MOTOR VEHICLE SAFETY STANDARD NO. 110, "TIRE SELECTION AND RIMS." THESE VEHICLES WERE SOLD WITHOUT THE REQUISITE LOAD CARRYING CAPACITY MODIFICATION LABELS.
Risk: A DRIVER MAY OVERLOAD A VEHICLE WHICH MAY INCREASE THE RISK OF A CRASH.
Fix: DEALERS WILL MAIL TO CONSUMERS THE CORRECTED LABEL OR THE CUSTOMER WILL HAVE THE OPTION FOR DEALERS TO INSTALL THE LABEL FREE OF CHARGE. THE SAFETY RECALL BEGAN ON MAY 27, 2010. OWNERS MAY CONTACT SOUTHEAST TOYOTA AT 1-800-301-6859.
NHTSA Campaign: 06V188000
Component: STEERING:ELECTRIC POWER ASSIST SYSTEM
THE INTERMEDIATE SHAFT AND SLIDING YOKE IN THE ELECTRIC POWER STEERING SYSTEM CAN CRACK WHEN LARGE FORCES ARE APPLIED AND THE CONNECTION MAY SEPARATE OR THE SLEEVE MAY FRACTURE.
Risk: THIS COULD RESULT IN A LOSS OF STEERING CONTROL OF THE VEHICLE.
Fix: TOYOTA WILL REPLACE THE STEERING INTERMEDIATE SHAFT AND INSPECT THE SLIDING YOKE AND REPLACE IT IF NECESSARY. THE RECALL BEGAN ON JUNE 9, 2006. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331.
Owner-Reported Issues (20 of 1438)

Your brakes completely failed while your daughter was driving through a 4-way stop, leaving her unable to stop the car—fortunately, your father was able to guide her to safety. There was a recall issued in 2016 for the Brake Actuator Assembly (the part that controls your brakes), but Toyota says the recall has expired. The brake system should be inspected and the failed brake actuator assembly likely needs to be replaced to restore safe braking.

When you brake while going over a bump, your Prius lurches forward and the traction control light comes on. During this lurch, your brakes temporarily fail for 1-2 seconds before re-engaging and allowing you to stop. Your dealer mentioned there's an ECM reprogramming fix available, though they're uncertain if it applies to your 2006 model since it was officially released for 2008-2009 models.

Air Bags →1 issue
You were hit on your driver's side door by another vehicle traveling at 35 MPH while you were turning left at less than 3 MPH in an intersection. Despite the serious impact, your airbags did not deploy, and you sustained injuries including a broken rib, rotator cuff damage, whiplash, and back injuries that required medical attention. The airbag system may have failed to sense the crash impact or trigger deployment when it should have.

Electrical System,service Brakes →1 issue
Your Prius's brake system may fail suddenly when the 12-volt battery ages or weakens, especially in cold weather or if the electrical system is heavily used. You'll hear an alarm and see ABS and VCS warning lights before the front brakes stop working, followed by rear brakes locking up. Installing a new 12-volt battery usually resolves the issue, but there's no dashboard warning to alert you when the battery is too weak for safe braking. The fix likely involves updating the brake control unit's electrical design to handle normal battery aging better, and adding a battery condition warning to your dashboard.

Electrical System,exterior Lighting,engine →1 issue
Your dashboard lights, including the gas gauge, speedometer, and transmission indicator, stopped working when you started your car. Additionally, your car wouldn't turn off, forcing you to complete your errands with the engine running. This appears to be a known issue affecting multiple Prius owners, potentially caused by a failed diode on the instrument cluster circuit board, which may require replacement of the entire dashboard display unit.

Service Brakes, service Brakes, Hydraulic →1 issue
Your car's ABS sensor (part of the brake system) failed while you were driving on the highway. The ABS system helps prevent wheel lockup during hard braking, so this failure could have made it harder to stop safely. This sensor should typically last much longer than it did. Fixing this will likely involve replacing the faulty ABS sensor.

Electrical System,exterior Lighting →1 issue
Both of your headlights went out while driving at night, nearly causing a crash. After turning the car off and back on, they came back on, but the driver's side headlight keeps going out intermittently. You also notice the speedometer reads 4-5 mph higher than your actual speed. The dealership quoted over $2,400 to replace both headlight bulbs and wiring, with no guarantee the fix will work. This issue is costing you money since you work night shifts and can't reliably drive your car.

Exterior Lighting, unknown Or Other →1 issue
Your Prius headlights aren't working properly, and a warning light has been on since shortly after you bought the car. Even though Toyota and a technician have replaced the headlights and relay fuses, the problem persists. This is a safety concern because faulty headlights increase accident risk at night and can result in traffic citations and failed vehicle inspections. The fix may require diagnosis of the electrical circuit or replacement of additional components like the headlight control module or wiring harness.

Electrical System,vehicle Speed Control,wheels →1 issue
Your instrument cluster display (odometer, fuel gauge, and speedometer) doesn't light up consistently, and when it's dark, your cruise control and warning lights also stop working. This happens intermittently rather than all the time. The issue is caused by defective capacitors on the display circuit board. Fixing this will likely require replacing the capacitors on the instrument cluster circuit board or replacing the entire cluster.

Electrical System →1 issue
Your dashboard lights and instrument panel randomly fail to turn on when you start your vehicle, even though the car runs normally. This makes it difficult to see your speed, fuel level, and warning lights while driving. The fix will likely involve diagnosing the electrical connection to your dashboard or replacing a faulty dashboard circuit board.
