NHTSA Campaign: 12V537000
Component: STEERING
Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 5, 2003, through March 30, 2009. The affected vehicles have a steering intermediate extension shaft assembly built with metal splines that complete the connection from the steering wheel to the steering gear. These splines may have been manufactured with an insufficient hardness.
Risk: Over time, the splines may wear and eventually fail resulting in a loss of steering ability, increasing the risk of a vehicle crash.
Fix: Toyota will notify owners, and dealers will inspect the extension shaft and replace it if it is one with insufficient hardness, free of charge. The recall began during December 2012. Owners may contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331.
NHTSA Campaign: 12V536000
Component: ENGINE AND ENGINE COOLING
Toyota is recalling certain model year 2004-2009 Prius vehicles manufactured August 6, 2003, through March 30, 2009 and FCHV-adv vehicles manufactured December 12, 2008, through September 13, 2011. During manufacturing, a scratch may have occurred inside of the electrically driven water pump at the coil wire. The coil wire may corrode at the scratched portion.
Risk: The corroded coil wire may break and the water pump could stop. The corroded coil wire could cause a short circuit between coil wires and a possible open fuse, creating a stall-like condition of the hybrid system while the vehicle is being driven. This may increase the risk of a vehicle crash.
Fix: Toyota will notify its owners, and dealers will replace the electric water pump, free of charge. The recall began on January 17, 2013. Owners may contact their Toyota dealer or the Toyota Customer Experience Center at 1-800-331-4331.
NHTSA Campaign: 09V388000
Component: VEHICLE SPEED CONTROL:ACCELERATOR PEDAL
TOYOTA IS RECALLING CERTAIN MODEL YEAR 2004-2010 PASSENGER VEHICLES. THE ACCELERATOR PEDAL CAN GET STUCK IN THE WIDE OPEN POSITION DUE TO ITS BEING TRAPPED BY AN UNSECURED OR INCOMPATIBLE DRIVER'S FLOOR MAT.
Risk: A STUCK OPEN ACCELERATOR PEDAL MAY RESULT IN VERY HIGH VEHICLE SPEEDS AND MAKE IT DIFFICULT TO STOP THE VEHICLE, WHICH COULD CAUSE A CRASH, SERIOUS INJURY OR DEATH.
Fix: TOYOTA FILED AN AMENDED DEFECT REPORT ON NOVEMBER 25, 2009, STATING THAT DEALERS WILL MODIFY THE ACCELERATOR PEDAL AND, ON CERTAIN VEHICLES, ALTER THE SHAPE OF THE FLOOR SURFACE UNDER THE PEDAL. THESE CHANGES ADDRESS THE RISK OF PEDAL ENTRAPMENT DUE TO INTERFERENCE WITH THE FLOOR MAT. REDESIGNED ACCELERATOR PEDALS WILL BECOME AVAILABLE BEGINNING IN APRIL 2010 AND DEALERS WILL REPLACE ANY MODIFIED PEDAL WITH THE NEW PEDAL IF DESIRED. ALSO, DEALERS WILL REPLACE ANY GENUINE TOYOTA OR LEXUS ALL-WEATHER FLOOR MATS WITH REDESIGNED ALL-WEATHER MATS, OR REPURCHASE THE PREVIOUS MATS FROM OWNERS WHO DO NOT WANT THE NEW ONES. ADDITIONALLY, SOFTWARE MODIFICATIONS WILL BE INSTALLED ON CAMRY, AVALON AND LEXUS ES 350, IS 350 AND IS 250 MODELS THAT WILL ENSURE THAT THE BRAKE OVERRIDES THE ACCELERATOR IN THE EVENT BOTH BRAKE AND ACCELERATOR PEDALS ARE APPLIED. TOYOTA WILL BEGIN MAILING LETTERS TO OWNERS ON DECEMBER 31, 2009. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331, LEXUS AT 1-800-255-3987.
NHTSA Campaign: 06V096000
Component: AIR BAGS
ON CERTAIN VEHICLES, DUE TO IMPROPER ASSEMBLY OF THE AIR BAG INFLATOR, WHICH IS USED IN THE SIDE AIR BAG, THE CURTAIN SHIELD AIR BAG, AND THE KNEE AIR BAG ASSEMBLY, SOME INFLATORS WERE PRODUCED WITH AN INSUFFICIENT AMOUNT OF THE HEATING AGENTS NECESSARY FOR PROPER AIR BAG DEPLOYMENT. IN THIS CONDITION, THE EXPANSION FORCE OF THE GAS MAY BE INSUFFICIENT TO PROPERLY INFLATE THE AIR BAG WHEN THE SRS SYSTEM IS ACTIVATED DURING A CRASH.
Risk: THIS MAY INCREASE THE RISK OF INJURY TO THE OCCUPANT IN THE INVOLVED SEATING POSITION IN THE EVENT OF A CRASH.
Fix: DEALERS WILL REPLACE THE SPECIFIC SRS AIR BAG. THE RECALL BEGAN ON APRIL 6, 2006. OWNERS MAY CONTACT TOYOTA AT 1-888-270-9371, SCION AT 1-866-548-1851, OR LEXUS AT 1-800-255-3987.
NHTSA Campaign: 04V558000
Component: EXTERIOR LIGHTING:BRAKE LIGHTS:SWITCH
CERTAIN PASSENGER VEHICLES MAY BE EQUIPPED WITH AN IMPROPERLY DESIGNED BRAKE LIGHT SWITCH. A SILICON OXIDE BUILD-UP OCCURS ON THE CONTACTS INSIDE THE BRAKE LIGHT SWITCH, WHICH CAN MAKE IT INOPERABLE.
Risk: IF THE SWITCH IS INOPERABLE, THE BRAKE LIGHTS WILL NOT ILLUMINATE AND CAN INCREASE THE RISK OF A CRASH.
Fix: DEALERS WILL REPLACE THE BRAKE LIGHT SWITCH. THE RECALL BEGAN DURING DECEMBER 2004. OWNERS SHOULD CONTACT TOYOTA AT 1-800-331-4331.
NHTSA Campaign: 06V188000
Component: STEERING:ELECTRIC POWER ASSIST SYSTEM
THE INTERMEDIATE SHAFT AND SLIDING YOKE IN THE ELECTRIC POWER STEERING SYSTEM CAN CRACK WHEN LARGE FORCES ARE APPLIED AND THE CONNECTION MAY SEPARATE OR THE SLEEVE MAY FRACTURE.
Risk: THIS COULD RESULT IN A LOSS OF STEERING CONTROL OF THE VEHICLE.
Fix: TOYOTA WILL REPLACE THE STEERING INTERMEDIATE SHAFT AND INSPECT THE SLIDING YOKE AND REPLACE IT IF NECESSARY. THE RECALL BEGAN ON JUNE 9, 2006. OWNERS MAY CONTACT TOYOTA AT 1-800-331-4331.
Owner-Reported Issues (20 of 610)
Service Brakes →7 issues
Your car's brake system is displaying error codes C1256 and C1391, which indicate low pressure in the ABS (anti-lock brake) system and an abnormal leak. Your brakes may not function normally if this issue isn't addressed. The problem likely involves a leak in the ABS pressure system that needs to be located and repaired.

Your brake actuator assembly failed in 2021, but Toyota's limited warranty enhancement program (ZG1) only covered failures reported between April 2016 and December 2017. Toyota declined to repair your brakes under this program since your failure occurred after the deadline. The brake actuator is critical to your braking system, and you believe Toyota should have issued a full recall instead of using a time-limited warranty program.

Your brake actuator has failed, which is a critical component of your braking system. The dealer informed you that Toyota previously covered this part under warranty but no longer does. You discovered that Toyota extended warranty coverage for this known defect on 2010-2015 Prius models until August 2021, but your 2004 model doesn't qualify. Repairing this will cost around $4,000 and will likely require replacement of the brake actuator assembly.

Your car's brakes occasionally fail to respond immediately when you press hard on them, causing the calipers to release completely. When this happens, you have to release the brake pedal and press it again to stop, which doesn't always give you enough time in an emergency—as evidenced by a collision that occurred when you couldn't repeat this cycle fast enough. The fix will likely involve inspecting and possibly replacing brake system components such as the calipers, master cylinder, or brake lines to restore proper brake pressure response.

While driving, your brakes suddenly lost most of their stopping power—you felt like you only had about 10% of normal braking ability. Your brake warning light, ABS light, and stability control light all came on at the same time, and a buzzer sounded. The brakes felt like they were working manually without power assist. This will likely require inspection of your brake system's hydraulic pressure, ABS module, and brake fluid level.

Your car's anti-lock braking system (ABS) has stopped working. The ABS won't build up pressure, which means the anti-lock feature is no longer functioning. While your regular brakes should still work, you've lost the safety benefit of ABS preventing wheel lockup during hard braking. A mechanic will need to diagnose and repair the ABS hydraulic system, which may involve replacing the ABS pump, module, or related sensors.

Electrical System →4 issues
Your headlights are turning off randomly while you're driving, starting with the driver's side and now affecting both sides. Both headlights have gone out simultaneously twice at night, creating a serious visibility hazard. Temporarily turning all lights off and back on restarts them, but the problem happens repeatedly every few minutes once it starts. This issue has been getting progressively worse over several months. The fix will likely involve diagnosing the electrical circuit controlling the headlights, which may require checking the wiring, connectors, relay, or headlight switch.

Your odometer stops working at 299,999 miles, preventing you from tracking maintenance intervals and other mileage-based needs. Additionally, your entire digital dashboard occasionally goes dark, leaving you without a speedometer or fuel gauge, making the vehicle unsafe to drive. This is a known issue affecting 2004-2005 Prius models that Toyota acknowledges but won't repair due to your ownership duration. The fix will likely involve replacing the instrument cluster or dashboard display module.

Your drive selector lever won't stay in the Drive position during hot weather—it pops back out unless you hold it down manually. The problem is intermittent and disappears when temperatures cool, forcing you to drive one-handed to keep the car in gear, which is unsafe. Replacing the drive selector may help, though the issue recurred once after your 2022 replacement.

Exterior Lighting →3 issues
Your left headlight intermittently turns off while driving on the freeway in dark, rainy conditions, but comes back on when you toggle the headlights off and back on. The bulb itself isn't burned out. This electrical issue could be caused by a loose connection, corroded socket, or a failing headlight relay or switch.

Your instrument cluster display (dashboard lights and gauges) keeps going out intermittently, leaving you unable to see your speed, fuel level, or warning lights. The problem has happened multiple times over several months and temporarily resolves itself, but the dealership identified an electrical short in the instrument cluster and had to order a replacement part. You had to pay upfront for the repair because the mechanic warned it might fail again.

Steering →2 issues
While making a gentle turn at low speed, you heard a clicking noise and your steering wheel suddenly spun freely without controlling the front wheels, leaving you unable to steer. This caused a minor collision with debris and bumper damage. A mechanic found that the main bolt connecting the intermediate steering shaft to the steering rack had come loose and reattached it. The mechanic noted that your vehicle had previously had two related Toyota recalls completed for the steering shaft and intermediate steering shaft extension. Reattaching or replacing the steering shaft-to-rack bolt connection should resolve this issue.

Your power steering warning light is turning on in your 2004 Prius. The vehicle was already repaired under a Toyota recall for this issue, but the warning light came back on after the repair was completed. This suggests the recall fix didn't fully solve the problem. The repair may require further diagnosis of the power steering system, possibly including replacement of the steering control module or related electrical components.

Electrical System,engine →1 issue
Your 2004 Prius stalled while driving at 30 mph after warning lights appeared on your dashboard, and the instrument panel went dark. You lost power steering but were able to safely pull over; however, the car wouldn't restart. A mechanic found that a wiring cable connector had melted onto the battery, damaging the engine, and the fuel pump also needed replacement. This repair involved replacing the melted connector, repairing battery connections, and installing a new fuel pump.

Vehicle Speed Control, unknown Or Other →1 issue
Your 2004 Prius has two problems: the ABS accumulator (part of the braking system) has failed, causing your brakes to lock up even after getting new brake pads, and the multi-function display screen in your dashboard has gone completely blank, making it unsafe to drive. Both issues appear to be manufacturing defects, and you're asking Toyota to cover the repair costs since previous recall notices may have expired before the problems occurred. Fixing the ABS accumulator will require replacing the accumulator unit itself, and the display issue will likely require a dashboard display replacement or repair.

Electrical System,exterior Lighting,unknown Or Other →1 issue
Your instrument cluster lights keep going out, which means you can't see your fuel level, speed, or warning lights while driving. This prevents you from knowing how fast you're going or if there's a problem with your car that needs immediate attention. The fix will likely involve replacing the instrument cluster or its backlight bulbs/LEDs.
