THE CONTACT STATED THAT THE BRAKE AND ACCELERATOR PEDALS WERE TOO CLOSE IN PROXIMITY, CAUSING HIM TO ERRONEOUSLY DEPRESS THE ACCELERATOR AND THE BRAKES SIMULTANEOUSLY. THE VEHICLE WAS TAKEN TO THE DEALER WHERE THE TECHNICIAN STATED THAT THE VEHICLE WAS PERFORMING TO STANDARDS. THE MANUFACTURER WAS MADE AWARE OF THE FAILURE. THE VEHICLE WAS NOT REPAIRED. THE FAILURE MILEAGE WAS 16,000 AND THE CURRENT MILEAGE WAS 18,000.
