When slowing to a stop with foot off gas pedal and on the brake pedal engine suddenly revs to approximately 7K rpms. Mashing brake pedal and turning off engine is only way to control the vehicle. Restarting motor has returned it to normal operation until last occurrence when it took about 5-6 restarts before it returned to normal operation. This has been happening since car was maybe 1 year old. However, it goes for extended periods of 6 months or more without it occurring. It occurs with absolutely no warning.

The contact’s daughter owns a 2016 Toyota Rav4. The contact stated that while the vehicle was idle, the warning message “System Failure” illuminated and the vehicle shuddered, shut off, and lost electrical power. The contact’s daughter shifted into Park. The vehicle was towed to the dealer who diagnosed the failure as a loose vacuum hose and retched fuel system. The dealer secured the vacuum hose; however, the failure reoccurred while she was at a complete stop. The vehicle was towed to the dealer who diagnosed the failure as a contaminated charcoal canister. The dealer replaced the canister; however, the failure reoccurred. The vehicle was towed to the dealer and it was stated that fuel was leaking onto the charcoal canister. The charcoal canister was replaced for the second time; however, the failure persisted. The vehicle was taken back to the dealer for a 4th time. The dealer sent a full diagnostic report to the manufacturer. The manufacturer sent engineers, field experts, and a district manager who took the vehicle apart and diagnosed that the charcoal canister and the evaporative emission control system including the gas tank needed to be replaced. The contact stated that the vehicle was repaired, and the gas cap was tightened. However, the failure persisted. The vehicle was towed to the dealer three more times. The charcoal canister was replaced but the failure persisted. The contact stated that failure would occur when the vehicle was idle at a complete stop. The contact referenced NHTSA Campaign Number: 20V682000 (Fuel System, Gasoline). The contact stated that the manufacturer opened a case and had been assigning the case to different district managers. The vehicle was not repaired. The failure mileage was approximately 1,200.

I USED TO TAKE FULL TANK GAS EVERYDAY SINCE BOUGHT THE CAR . AFTER DRIVING 1 YEAR MY OIL TANK GIVING ME PROBLEM. MY ENGINE WAS SUDDENLY SHUT DOWN AUTOMATICALLY AND 3 SIGN CAME UP. I STARTED THE CAR AGAIN AND IT WAS WORKING GOOD AND AFTER A WHILE IT SHUT DOWN AGAIN. I WENT TO MY DEALER ABOUT 5 MONTH AGO AND TOLD THEM THE STORY. THEY DIAGNOSE MY CAR AND FOUND NOTHING SO I TOOK MY CAR BACK FROM DEALER SHOP. NEXT DAY SAME PROBLEM HAPPENS AND I WENT BACK TO THE DEALER. . THIS TIME THEY FOUND MY BATTERIES ALMOST FINISHED AND I HAVE PROBLEM WITH MY AIR SENSORE . SO I FIXED BOTH OF THEM AND PAID ALMOST 1400 DOLLAR WHERE PARTS COST WAS ONLY 300 AND REST WAS FOR 5 HOURS COMPUTER DIGONOATIC. ANYWAY, I THOUGHT MY PROBLEM IS FINISHED BUT IT'S STILL THERE. NOW MY GASOLINE COMING OUT FROM TANK AND I AM GETTING SMELL FROM INSIDE. I AGAIN WENT BACK TO DEALER, THIS TIME THEY KEEP MY CAR FOR 1 DAY AND NEXT DAY THEY TELLING ME THAT I HAVE TO CHANGE MY ENTIRE GASOLIN TANK BECAUSE I USED TO TAKE FULL TANK GAS,WHICH WILL COST AROUND 2000 DOLLAR!! I WAS VERY MAD WITH THE MANAGER BECAUSE IN FIRST TIME THEY COULDNOT EVEN FIND MY REAL PROBLEM AND CHANGED MY NEW BATTERIES AND AIR SENSOR! NOW THEY TELLING ME IT'S VERY DANGEROUS TO DRIVE WITHOUT FIXED OIL TANK BUT I DIDNOT FIXED IT BECAUSE IT MUST BE FACTORY DEFECT, WHY SHOULD I PAY FROM MY POCKET. I CALLED TOYOTA CORPORATE OFFICE AND TOLD THEM ABOUT MY ISSUE. THEY TOLD ME SINCE I HAVE NO WARRENTY, THEY CAN'T FIXED IT FREE FOR ME.AND THERE ARE NO RECALL ABOUT MY GAS TANK!

TAKATA RECALL; WHILE ATTEMPTING TO BACK OUT OF A DRIVE WAY, THE ACCELERATOR STUCK CAUSING THE CAR TO HIT A BUSH AND A HOUSE. AFTER RECOVERING AND ATTEMPTING AGAIN, THE CAR ACCELERATOR STUCK A SECOND TIME CAUSING THE CAR TO HIT ANOTHER BUSH.
