Your seat belt latch comes undone when you lean forward, which happened twice after you bought the vehicle. During a car accident, your seat belt unlatched completely, and you were thrown into the windshield, causing head, neck, and shoulder injuries. The seat belt latch mechanism needs to be inspected and likely replaced to ensure it holds securely during normal driving and in a crash.
