Your driver's side seat belt buckle receptacle doesn't always click properly and the belt can come undone while you're driving. When this happens, you may have difficulty getting the belt to latch back in. The problem has been getting worse since late March, and your Toyota dealer hasn't been able to fix or replace the part yet. The fix will likely involve replacing the seat belt buckle assembly on the driver's side.

Your seatbelts are malfunctioning in multiple ways: they lock up before you can buckle them, fail to retract after unbuckling and hang out the door (draining your battery), and get twisted and stuck in the plastic guide at the top of the door. This has been an ongoing problem for years, and one seatbelt became so stuck it required cutting to free it, leaving it extremely difficult to use now. You'll need to have the seatbelt assemblies inspected and replaced to restore proper function and safety.
