While slowing down in heavy traffic by pressing the brake pedal, your engine suddenly accelerated uncontrollably, racing at high RPM and propelling the car forward like the brakes had no effect. Despite applying more brake pressure, you couldn't stop the car and rear-ended the vehicle in front of you; the engine continued racing even after the collision until you turned off the ignition. This suggests a potential failure in the engine control computer or throttle system that overrode your braking input.

I BELIEVE THAT THE CLEARANCE BETWEEN BRAKE PEDAL AND ACCELERATOR ON THE TOYOTA RAV 4 2001 AND PERHAPS OTHER YEARS IS INADEQUATE. A WHILE BACK MY FOOT BECAME TRAPPED BETWEEN THE TWO PEDALS AND I WAS UNABLE TO APPLY THE BRAKE. A SMALL ACCIDENT OCCURRED. I SUSPECT THAT THE PROBLEM IS AGGRAVATED BY THE HEIGHT DIFFERENCE OF THE TWO PEDALS.. *TR

THE CONSUMER LOSS CONTROL OF VEHICLE GOING 65MPH ON A STRAIGHT PATH, THE VEHICLE WAS RAPIDLY THROWN TO THE RIGHT AND LEFT SEVERAL TIMES AFTER IT ROLLED OVER 8-10 TIMES.
VIBRATION/RATTLE AT ALL SPEEDS FROM WINDSHIELD AREA OF VEHICLE. APPEARS TO BE FROM PLASTIC COWLING AROUND WINDSHIELD AND WIPERS. MAY BE DUE TO IMPROPER MOUNTING OF EITHER WINDSHIELD OR COWLING. MAY INTERFER WITH WIPER ACTION.*AK
