Your child pulled the second-row passenger seatbelt (next to the window) so tightly that it wrapped around their neck and became stuck. Turning the car off and on again didn't release it, so you had to cut the seatbelt with scissors to free them. The seatbelt retractor mechanism appears to have jammed and failed to allow normal release.
